Air Canada caught a licensing violation, reported it voluntarily and removed the pilot immediately. Every pilot at Air Canada completes mandatory recurrent training every six months, and competency checks happen twice a year, so safety was never actually at risk. A full audit found zero other violations, and administrative practices have since been tightened.
An Air Canada captain flew more than 900 flights over 16 years using counterfeit documents, and neither Transport Canada nor Air Canada caught it. This was a massive, years-long failure of oversight on domestic and international routes carrying real passengers. Authorities have now filed charges, but the damage to public trust in aviation safety oversight is already done.
